  • You are able and willing to sign informed consent
  • You are 18 years old or older
  • You have a confirmed diagnosis of prostate cancer, head and neck cancer, lung cancer, or a brain tumor
  • Your treatment is meant to cure you (curative intent)
  • Your health is good enough for treatment (ECOG score 0 or 1)
  • You expect to live 18 months or longer and can complete required long-term online questionnaires
