- You must have been in the hospital for possible or confirmed COVID-19.
- You must have had a biological sample test showing SARS-CoV-2 (the COVID-19 virus) positive, OR
- If your test was negative, you still must have strong signs of COVID-19 based on symptoms and chest imaging (scan).
- If you were not admitted, you may still qualify if you left the emergency department and had a SARS-CoV-2 positive sample.
