- You are 18 years or older
- You have an incurable cancer that is either rare or has a rare tumor change (molecular aberration)
- You are currently on a targeted cancer treatment approved by Health Canada that is not paid for nationally
- Your general condition is okay (ECOG 0 to 2) and you likely have at least 12 weeks to live
- You have adequate blood and organ function, as your clinician decides
- You can complete quality-of-life questionnaires and give informed consent
