  • You have been diagnosed with prostate cancer confirmed by imaging (scan results).
  • You are at least 18 years old.
  • You are planning to enroll in a prostate cancer clinical trial.
  • You can understand, sign, and return the consent form.
  • You can do regular electronic reporting (using phone/computer).
  • Your health functioning is not extremely limited (ECOG score 4).
