  • You are 18 years old or older.
  • You had a heart attack, unstable chest pain, or a procedure like a stent or bypass in the last 2 years (at least 6 months ago), OR you are at high risk due to taking daily medicines for blood pressure, cholesterol, or diabetes.
  • You signed a consent form to join the study.
  • You live in the area near a hospital that is part of this study.
