  • You identify as Black or African Diaspora
  • You are 18 or older
  • You are enrolled in NYU Langone's Faculty Group Practice for pregnancy care
  • You plan to give birth (or already gave birth) at NYU Langone Tisch hospital
  • You joined the Holistic Care Partner program while pregnant or within 6 weeks after giving birth
  • You are not pregnant with twins or triplets, and your baby does not have a major health issue requiring special care
