This trial tests whether a drug called an immune checkpoint inhibitor (PD-1 blocker) can help lower the risk of cancer returning after surgery. It is for people with a specific type of colorectal cancer (dMMR or MSI-H) that had certain high-risk features.

Who can take part
- You had surgery to remove colorectal cancer that is stage II (T3 or T4, no lymph node spread).
- Your tumor tested positive for a genetic marker called dMMR or MSI-H.
- Your cancer had at least one high-risk feature, such as being T4, having blood vessel or nerve invasion, being poorly differentiated, or causing a blockage or hole in the bowel before surgery.
- Scans (CT, MRI, or PET) before or after surgery showed no signs of cancer spread.
- You are between 18 and 80 years old and generally able to take care of yourself (ECOG 0 or 1).
- You have not had prior chemotherapy, immunotherapy, or radiation for this cancer.
