This trial tests whether an antibiotic (amoxicillin) can reduce chronic low back pain in people who have a specific type of bone change (Modic type I) after a past herniated disc. If you qualify, you would receive either the antibiotic or a placebo (inactive pill) for several weeks.

Who can take part

  • You are between 18 and 65 years old.
  • You have had low back pain for more than 6 months, and it started after a herniated disc that was confirmed by MRI.
  • Your average pain level in the last 2 weeks is at least 5 out of 10.
  • You have a specific type of bone change (called Modic type I) near the old herniated disc, seen on an MRI.
  • You answer 'Yes' to at least 2 of these: pain gets worse with exercise, pain wakes you at night, turning in bed hurts, or morning pain.
  • You are not allergic to penicillin, not pregnant, and have not had antibiotics or back surgery in the last month.
