This trial is testing a new, enhanced home-delivered meal program for older adults. It aims to see if these meals improve health and well-being for people who are already eligible for Meals on Wheels and are at nutritional risk.

This trial tests whether virtual care after surgery helps older adults with frailty recover at home instead of going to a nursing home or rehab. You'll get check-ins via video or phone to support your recovery.
