Who can take part

  • You are 18 years or older
  • You have a phone or email we can use to contact you
  • You have a primary care doctor at Ascension
  • You've filled at least 2 prescriptions for a statin in the last year, with 1 in the past 6 months
  • Your medication adherence has been less than 80% in the last year (meaning you missed more than 20% of doses)
  • You've agreed to get messages from Ascension electronically
