Who can take part

  • You are 65 years or older and live at home or in assisted living, not in a nursing home.
  • You came to the emergency room because you fell, and your fall was not caused by fainting, a car accident, or an assault.
  • Your fall was not caused by a serious illness like a stroke or heart attack.
  • You will be going home or to assisted living or rehab after your visit (not staying in the hospital).
  • If you are unable to make decisions for yourself, a family member or friend can help with consent.
