Who can take part

  • You are between 60 and 80 years old.
  • You live in the community (not a nursing home or hospital).
  • You plan to stay at your current address for the next 12 months.
  • You can walk about 50 meters by yourself (a cane is okay).
  • You have fallen at least once in the last 2 years, OR you are worried about falling.
  • You are willing to join a 12-month fall prevention program and follow-up visits.
