- You are not up to date on colorectal cancer screening (such as colonoscopy, stool tests, or Cologuard).
- You have a primary care doctor at a Penn Internal Medicine or Family Medicine practice.

- You have never had colorectal cancer, a colectomy (colon removal), or stage 4 cancer.
