- You or your family caregiver must be 18 or older
- You must have a disability that requires a 4-week stay in a special transitional care unit at Pôle MPR St Hélier
- Your primary caregiver must be a family member or friend, not a paid professional

- You cannot be pregnant, breastfeeding, or currently in a severe psychiatric crisis
