Who can take part

  • You or your family caregiver must be 18 or older
  • You must have a disability that requires a 4-week stay in a special transitional care unit at Pôle MPR St Hélier
  • Your primary caregiver must be a family member or friend, not a paid professional
  • You and your caregiver must have social security coverage or similar benefits
  • You cannot be pregnant, breastfeeding, or currently in a severe psychiatric crisis
