  • You have been treated for squamous cell cancer in the head and neck
  • You received radiation therapy plus cetuximab (an anti-cancer antibody)
  • You received cetuximab either with chemotherapy or as part of specific cancer treatment plans
  • You have also been treated with a targeted therapy (medicine aimed at cancer targets) as part of a clinical study
  • Your cancer is not a different type of head and neck cancer
