This trial studies whether blocking PD-1 (an immune “brake”) along with niraparib (a DNA-repair blocker) can shrink or control recurrent/progressive uterine carcinosarcoma after prior chemotherapy. It’s for people whose cancer has grown again and who meet specific health and lab requirements.

Who can take part

  • You have recurrent or progressive uterine carcinosarcoma confirmed by expert pathology review.
  • Your cancer has gotten worse after at least one prior platinum chemotherapy treatment (including possibly after surgery).
  • You have had 1 or fewer prior cancer chemotherapy regimens specifically for carcinosarcoma (not more than 3 total cytotoxic chemo courses allowed).
  • You must have stored tumor tissue available from diagnosis or relapse (so they can test it).
  • You must be well enough to take pills and your key blood counts, liver, and kidneys must meet study limits.
  • You must not have had prior niraparib/PARPi or prior anti–PD-1/PD-L1 treatment.

Study of chemotherapy and pembrolizumab with or without nesuparib for endometrial cancer

This trial tests a chemotherapy combination (carboplatin and paclitaxel) plus the immunotherapy drug pembrolizumab, followed by maintenance pembrolizumab either alone or with a PARP inhibitor called nesuparib. It is for women with advanced or recurrent endometrial cancer that is MMR-proficient (a type where the cells have a normal ability to fix DNA).
