This trial tests a new treatment approach for people with ovarian cancer that has started to progress in a few spots (up to 3) after taking a PARP inhibitor. It combines targeted radiation (SBRT) to those spots, followed by another PARP inhibitor (niraparib), to see if this can control the cancer further.

Who can take part
- You are 18 years or older.
- You have ovarian, fallopian tube, or primary peritoneal cancer that has been confirmed by a biopsy.
- Your cancer started growing again while you were taking, or after you finished, a PARP inhibitor (like Lynparza or Zejula). That PARP inhibitor was your most recent cancer treatment.
- You took a PARP inhibitor for at least 6 months, either as your first treatment or for recurrent disease.
- You have 3 or fewer spots of cancer that are growing (each smaller than 4 cm) that can be treated with focused radiation (SBRT).
- Your cancer can be seen on a CT or MRI scan (measurable disease).
- You are well enough to have a daily life without much help (ECOG 0 or 1).
