Who can take part
- You (or your legal representative) agree to join and sign the consent form
- You are at least 65 years old and have been taking at least 5 medications for 6 months or more
- You are currently in a hospital surgery department
- You can be followed for about 3 months after hospital care
- You have a “Trivalle” medication-risk score of 2 or higher, and you live in a nursing home or plan to go home after hospitalization
- You are not in another certain type of medication-intervention study, and you are not receiving palliative (comfort-focused) care
