Who can take part
- You are an adult (18 or older).
- You live in Oslo and will be going home (or to a short-term nursing home stay) after the hospital visit.
- You usually manage your own medicines at home (you may have some help from home-care nurses or family).
- You take at least 4 regular medicines from at least 2 different types (classes) of medicines.
- You can communicate in Norwegian or English.
- You (and the GP or home-care nurse involved, plus your next of kin/caregiver if involved) can agree to take part in writing.
