Who can take part
- Be between 18 and 70 years old when you agree to join
- Have a confirmed diagnosis of HCM, TAD, and/or RIHD and be seen at Cleveland Clinic’s main campus for a cardiac care plan
- Have access to a phone, computer, or tablet plus internet to answer emailed questionnaires
- Not plan to have heart surgery (open chest or catheter-based) for HCM or TAD in the next 9 months, or not have had such surgery already
- Be able and willing to follow the study steps and provide informed consent
