- You have been diagnosed with a primary immune deficiency
- You have had COVID-19, either confirmed by a test or considered likely based on your doctors’ judgment
- You do not have a secondary immune deficiency (a weaker immune system caused by another condition or treatment)
- You have not had a different coronavirus infection instead of COVID-19
