Who can take part

  • You had a heart transplant on or after January 1, 2015.
  • You were at least 18 years old at the time of your transplant.
  • You were not deceased within 48 hours after your transplant.
  • Your transplant was not a combined heart-lung transplant.
  • Your transplant was not a combined heart-kidney transplant, and it was not a re-transplant within 7 days of the last surgery.
