This trial studies pembrolizumab (an immunotherapy) in people with muscle-invasive bladder cancer who had a combined “trimodality” treatment and then achieved a complete response. The goal is to help control the cancer after the initial treatment is finished.

Who can take part
- You must be 18 or older
- Your bladder cancer must be muscle-invasive (about stages cT2 to cT4), with no spread to lymph nodes or distant sites (N0M0)
- You must have had full “trimodality” treatment, including the most complete TURBT surgery possible, and then had a complete response checked 28–90 days after treatment ended
- The cancer should be measurable on scans, and if a spot was in an area that was previously radiated, it must show progression there
- You should be well enough to do daily activities (ECOG 0 to 2) and your organs (blood, liver, kidneys) must meet lab requirements
- You should not have had prior treatment with immune checkpoint drugs like pembrolizumab, nivolumab, or atezolizumab
