Who can take part
- You are age 18 or older and currently admitted to the hospital ward (not a special higher-level unit).
- Your care team has you as a high-risk patient for needing the most aggressive available care (Full Care, with limited exceptions).
- You fit one of these situations: you had high-risk surgery, you have high-risk blood cancer or other cancer, you were admitted for infection from the emergency department, or your team marked you as high-risk.
- You are not being admitted to a unit with higher monitoring like ICU (intensive care) or similar higher-observation areas.
- You are not in comfort-focused care, and you are not in the lowest ‘life support/CPR limited’ status (Category III or IV).
