Who can take part

  • Your child is under 18 years old at the time the study enrolls
  • Your child was hospitalized for pneumonia, a skin/soft tissue infection, gastroenteritis (stomach bug), or a urinary tract infection
  • A parent/guardian can speak English or Spanish
  • There is not already a follow-up visit scheduled within 7 days after hospital discharge
  • There is no major complex long-term medical condition that makes follow-up especially complicated
  • The child does not have certain complicated forms of the illness (like chest tube for pneumonia, specific bladder/kidney issues for UTI, severe wound complications for skin infections, or hemolytic uremic syndrome for gastroenteritis)
