- You were diagnosed with cancer and were treated with the goal to cure it
- You were between 18 and 65 when you were diagnosed
- You were working (full-time or part-time, including being a student) at the time of diagnosis
- You are still on sick leave or long-term disability leave, but it has been less than 2 years
- You have not started returning to work yet since stopping because of cancer
- You live in Canada and can read and understand English or French
