Who can take part

  • You take five or more prescribed medications (or your primary care doctor thinks you may need this program).
  • You live at home, not in a nursing home.
  • You are registered with a primary healthcare center in rural areas of Västerbotten County (or a nearby county if needed).
  • You can speak and communicate in Swedish.
  • You do not have a confirmed major memory/thinking disorder (major neurocognitive disorder).
  • You are not receiving palliative (comfort-focused end-of-life) care.
