- You are pregnant and age 18 or older
- You are less than 20 weeks pregnant when you start (before 20 weeks + 6 days)
- You had type 2 diabetes before pregnancy, and your A1c is at least 6.5% at enrollment
- You have Medicaid insurance
- You can speak English or Spanish and understand the study enough to follow it
- You have a smartphone with internet, and you can meet study visits/activities
