Who can take part
- You are 65 years old or older.
- You have frailty level 4 to 7 (needing some help or having reduced strength/ability).
- You have type 2 diabetes—either you’ve been told you have it, you take diabetes pills, or your blood sugar test (HbA1c) is 6.5% or higher.
- You can understand the study, follow the visit plan, and sign informed consent (or have someone who can help if allowed by the study).
- You are NOT using insulin injections or an insulin pump.
- You do not have type 1 diabetes, and you don’t have major uncontrolled or very serious illnesses (such as severe heart, lung, liver, kidney, brain/mental health problems, or severe recent infection).
