Who can take part
- You have one or more stable chronic conditions (stroke, heart/coronary disease, type 1 or type 2 diabetes, chronic respiratory failure, Parkinson’s disease, or epilepsy).
- You are currently being followed by an advanced practice nurse (APN) for your condition.
- Your APN care is provided under the study’s practice setup (either within a healthcare facility or with physician referral/prescription when in private practice).
- You are covered by a social security health plan.
- You received the study information and signed consent (with the study team/investigator).
