  • You have at least one long-term health condition that is not cancer (like diabetes, heart disease, or arthritis).
  • You are 66 years or older.
  • You are cared for by a general practitioner in the Verona area Community Health Home.
  • Your doctor expects you to live longer than 12 months.
  • You do not get intensive home care visits more than once a week.
