- You can speak English or Spanish
- You were recently discharged from a Harris Health hospital emergency department (Ben Taub or LBJ)
- You are at imminent risk of becoming homeless
- You have Medicaid, Medicare, or no health insurance
- You do not have a court-appointed legal guardian/conservator
- You plan to stay in Texas for the next 6 months (not moving away)
