- You must have HIV
- You must be age 50 or older
- You receive HIV care through the Atrium Health Wake Forest Baptist Infectious Diseases Specialty Clinic
- You need to be eligible for the Ryan White Program (a HIV care assistance program)
- You must be able to answer consent and complete required questionnaires/assessments
- Your health measures suggest you are pre-frail or frail based on an electronic frailty index
