Who can take part

  • You’re an adult (older than 18).
  • You’ve had type 2 diabetes for at least 6 months.
  • Your recent blood sugar test (HbA1c) has been above 7% on at least two visits in the last year.
  • You can read and speak English or Spanish, and you’re willing to text with the study team.
  • You will give informed consent and plan to stay in care at the clinic for at least 12 months.
