Who can take part

  • Be at least 18 years old
  • Have type 2 diabetes with your A1c (a diabetes blood test) higher than 8.0
  • Currently receive care as a patient at the Family Healthcare Center at Reading Hospital Tower
  • Have your own smartphone with access to use it
  • Not be pregnant and not currently incarcerated
  • Be able to understand the study and give your own consent
