  • You’re 18 to 90 years old and have type 2 diabetes
  • You speak and understand English or Spanish
  • You’re taking at least one diabetes medicine by pill or injection
  • Your recent A1c blood test is 7.5% or higher
  • You can stay in the local area for the whole study and have a phone
  • You agree to attend all training and orientation sessions
