  • Your child has a serious health condition that needs monitoring at home.
  • Your child's heart doctor has said home monitoring might help their treatment plan.
  • Your child receives care at a hospital that is approved for this study (currently Children's Mercy Kansas City).
  • You are willing to use the CHAMP app on a smartphone or device.
