  • You are between 30 and 60 years old.
  • You have at least two long-term (chronic) health conditions, such as diabetes, high blood pressure, or arthritis.
  • You are able to use a phone or computer for online group activities.
  • You are not currently living in a nursing home or other institution.
  • You do not have a terminal illness or a disability that would stop you from answering questionnaires.
  • Your clinic or hospital has your current phone number or email address on file.
