  • You live in the community (not in a nursing home or similar facility)
  • You are 50 years old or older
  • You have at least 3 chronic conditions, affecting at least 3 different parts of the body
  • You have internet access and can use a device to communicate (phone, tablet, or computer)
  • You can give informed consent and understand the study using help if needed
  • You can read or write or you can use an email/electronic device with help from a caregiver
