Who can take part
- You are at least 15 years old.
- Your injury happened less than 48 hours before you arrived at the hospital.
- You came to the emergency room because of a serious injury (not just a minor bump or bruise).
- You were admitted to the hospital, died in the ER, or were transferred to another hospital from the ER.
- You do not have only an isolated arm or leg injury.
- You were seen by a doctor in the emergency room (not sent directly to a hospital room).
