  • You are 18 years old or older.
  • You were released from jail between February 2021 and July 2028.
  • You had a visit with a primary care doctor or a mental health provider at a participating health system in the last 3 years.
  • You currently live in Michigan or Minnesota.
  • You do not need a translator to participate in the study.
