- Your kidney function is very low (eGFR under 20) or you are on in-center hemodialysis.
- You receive kidney care at Mount Sinai, Einstein, Bellevue, NYU, or a participating dialysis center in New York.

- You speak English or Spanish.
- You are not already listed for a kidney transplant or planning to move from New York within a year.
