- 18 years or older
- Advanced kidney disease with a kidney function level of 25 or less
- Speak English or Spanish
- Under the care of a kidney doctor at Montefiore and seen in the last year
- Willing to work with a nurse practitioner for kidney education and care coordination
- Not currently being treated for active cancer
- Expected to live more than 1 year
- Not choosing only medical management without dialysis
- Not planning to move out of New York City in the next year
