  • You have been diagnosed with frailty (score 4-7 on the Clinical Frailty Scale)
  • You are able to talk and understand, and can sign a consent form yourself
  • You have not already signed an advance care plan or advance directive
  • Your eyesight and hearing are good enough to watch a video
