Who can take part

  • You have a tooth that was knocked out and the wear on its chewing surface is mild to moderate (not severe).
  • The tooth is a fully grown front tooth, canine, premolar, or first or second molar (not a wisdom tooth or baby tooth).
  • The tooth does not have any crowns or dental work that would change its natural wear appearance.
