Who can take part

  • You have been diagnosed with COPD for at least 1 year
  • Your COPD is stage 3 or 4 (severe to very severe)
  • You were recently in the hospital for at least 3 days due to a COPD flare-up
  • You have a personal mobile phone and know how to use it
  • You are alert and able to answer questions clearly
