  • You live in Philadelphia.
  • You are between 35 and 75 years old.
  • A doctor has told you that you have pre-diabetes, diabetes, high blood pressure, high cholesterol, or a BMI of 30 or more.
  • You have already had your first visit with a CEO community health worker.
  • You have at least one unmet social need, like trouble with housing, food, transportation, jobs, utilities, healthcare, health info, or social support.
  • You are willing to join the study and have regular access to a mobile phone or device.
