  • You have obesity (BMI 30–50 for non-Asian, or 27.5–50 for Asian individuals)
  • You weigh less than 400 pounds
  • You get care at or are willing to sign up with a participating community health center
  • You can understand and speak English
  • You are able to take part in the study for the next year (not planning to move away)
