Mobile Geriatric Team for Nursing Home Residents

This study will test a new team of specialized helpers who visit nursing homes to assist with complex medical and personal care needs. If you are 70 or older and have certain common older-age problems (like falls, memory issues, or trouble managing multiple health conditions), this mobile team may be able to help coordinate your care right where you live.

  • You are 70 years old or older
  • You have one or more of these issues: trouble planning future medical care, several health problems with frequent hospital visits, taking many medications, repeated falls, hard-to-manage behavior changes, recent weight loss, or memory problems
  • Your condition is stable (not needing urgent care right now)
  • You do not need treatments or tests that can only be done in a hospital
