If you are 70 or older and have certain common older-age problems (like falls, memory issues, or trouble managing multiple health conditions), this mobile team may be able to help coordinate your care right where you live.

- You have one or more of these issues: trouble planning future medical care, several health problems with frequent hospital visits, taking many medications, repeated falls, hard-to-manage behavior changes, recent weight loss, or memory problems
- Your condition is stable (not needing urgent care right now)
- You do not need treatments or tests that can only be done in a hospital
