This study looks at frailty (how easily people can manage daily life), kidney function, and overall primary care needs in older adults living in rural areas. You may be asked to complete repeated health checks and care questionnaires to see what patterns predict health and support.

Who can take part
- You are 65 years old or older
- Your kidney function is above a certain level (mGFR greater than 30)
- You can understand the study and sign written consent
- You should not be bedridden
- You should not be in end-of-life/palliative care
- You should be able to speak the study language and complete at least 60% of assessments
